CHE Trinity Health, Walgreens Announce Care Collaboration Agreement
CHE Trinity Health, one of the nation’s leading health care systems, and Walgreens (NYSE: WAG) (Nasdaq: WAG), the nation’s largest drugstore chain, has announced a clinical care collaboration agreement to pursue innovative models of care in select markets the health care delivery system serves. The collaborative model enables retail clinic collaborations, outpatient pharmacy services and expanded prescription and disease management programs that can help improve people's health while increasing consumers’ access and lowering overall healthcare costs.
Walgreens pharmacists and Healthcare Clinic nurse practitioners will work with CHE Trinity Health providers to further coordinate patient care, while giving people access to a broad range of health care services at Walgreens and CHE Trinity Health sites of care.

Recognizing that people want high-quality experiences at every location where they seek care – at the doctor's office, the clinic, the hospital or the pharmacy – the agreement calls for consistent collaboration between pharmacists, physicians and other health care providers. Walgreens and CHE Trinity Health aim to provide high quality population health services including patient education, medication therapy management and other wellness services.
To help further improve community health, the two organizations also plan to measure patient outcomes and to refine clinical care approaches.
