Mercy Home Care is a Medicare Certified, CHAP accredited program that began serving Siouxland in 1975. Our high quality, professional team of registered nurses, aides, physical, occupational, and speech therapists, and social workers provide a wide range of services individualized to meet each patient's needs. Mercy Home Care is often an ideal solution for those recovering from an injury or illness. We provide care to prevent hospitalization and, when necessary, a bridge between the hospital/nursing home and the patient's return to every day activities.
Mercy Home Care services include, but are not limited to: medication management, disease management, wound care, patient and family education related to current conditions and chronic illnesses, assistance with personal care, and more. Through comprehensive assessment and care plan development, our goal is to support the patient to remain safely in their homes. Some of the unique specialty programs we provide are: Hearts at Home (cardiac care, including congestive heart failure), diabetes management, infusion nursing, and fall prevention. In addition, in-home telemonitoring is available to monitor patient's blood pressure, pulse, and weight & oxygen, as ordered by the physician. To ensure excellence, all aspects of patient care are managed by the primary registered nurse or therapist assigned to each patient.
Our service area includes the following counties in: Iowa - Cherokee, Ida, Monona, Plymouth, Sioux and Woodbury; Nebraska - Burt, Cuming, Dakota, Dixon, Thurston, and Wayne; and South Dakota - Lincoln and Union. Mercy Home Care offers services 24-hours a day, seven days a week. Payment options include Medicare, Medicaid, Blue Cross/Blue Shield, and most third party payors.
