Transitions Day Program

Mercy Rehabilitations Transitions Day Program is designed for adolescents and adults with brain injuries, strokes or other neurological disorders. It enables patients to return to independent functioning and to be successful in returning to work, school, or running a home.

The Transitions program assists patients in improving their concentration, memory, strength, organization, safety/problem solving, and controlling emotions. Patients are trained to use strategies to assist them on performing tasks on a day-to-day basis.

The Transitions Program offers a team approach, which includes a physiatrist (a physician specializing in therapies), neuropsychologist, occupational therapist, speech therapist, physical therapist, nursing/case manager, the patient and the patient's family/support. The team and the patient make an individualized treatment plan and set goals, which are agreed upon by everyone.

The patient spends from 4-6 hours in the program 4-5 days a week. A schedule is set up to gradually increase the hours at work and/or school and reduce the number of hours in treatment as the patient improves.

Family members are strongly encouraged to be involved with the patient's rehabilitation. Family and patient conferences are held to discuss progress, continued needs for treatment, and goals.
