We are a 10-bed short-term skilled care unit designed to care for people who need daily medical attention and health supervision, but no longer need the full range of hospital services. Independence through training, teaching, and maximizing of the patient's functional potential are stressed. We serve a variety of illnesses including: stroke, orthopedics, IV therapy, brain injury, severe illnesses and many more.

One of the program's goals is to assist individuals in maximizing independence in their care and mobility within the limits of their capacity and to help reintegrate them into the community. Patients are progressively expected to take on day-to-day responsibility for themselves and their surroundings.

Our skilled care unit is covered by Medicare, and may also be included in other private insurance policies. In order to be eligible for payment, our facility is required to comply with extensive federal and state requirements.

Referral and Admission

Admission is based on guidelines defined by Medicare, the Joint Commission on Accreditation of Healthcare Organizations, and the State of Iowa.

Length of Stay
How long private insurance and Medicare will pay for skilled care depends upon the length of time your care needs comply with standards and criteria. Our Medical Social Worker/Discharge Planner will be working with you so you are aware of your projected discharge date. They also will help you plan so that determined services are in place at home when discharge day arrives. Adequate preparation for discharge requires early and active participation of the patient, the family and all others concerned.

Programs and Services

Rehabilitation Services - Occupational Therapy, Physical Therapy, Speech Therapy and daily activities are available as needed. A physician's order is required for these services. Medicare and /or your private insurance company may pay for these services. Family members are encouraged to attend therapy sessions.

Physical TherapyPhysical Therapy - This may include gait and transfer training, exercises to strengthen arms and/or legs, and activities to improve balance or coordination.

 

 

 

Occupational TherapyOccupational Therapy - This may include muscle strengthening, joint movement, endurance, and coordination. This may also include problems with perception and sensation.

This will assist in strengthening independence in daily living skills.

 

Speech PathologySpeech-Language Pathology - In speech therapy, patients work on thought processing skills such as organizing and sequencing; giving or understanding directions, talking on the telephone, reading signs, signing their name, etc. Speech -Language pathologists also assess and treat swallowing problems.


Recreation Recreation is essential to living. Since the purpose of the Skilled Care Unit is to help you return to your previous lifestyle, this is included in your therapy. Some activities are done individually and some in-group. An activity calendar is posted monthly in each room and on the board outside of the activity room. Families are strongly encouraged to be aware of the activity program and to participate.

SkilledSkilled coordinator initiates/coordinates the admission agreements for patients and verifies and obtains physician orders for admission to the Skilled Care Unit. The Coordinator conducts a pre-admission screen and is responsible for reviewing each case with physicians and caregivers. Medical Social ServicesMedical Social Services/ Discharge Planner will help plan your discharge. She/he will refer you to various community agencies that may be able to meet your needs or assist you in solving problems or concerns, which you may have.The medical social worker/discharge planner is also qualified to assist you or your family with alterations in emotional, physical and financial status to ensure continuity of care.

NursingNursing - Your care will be directed and carried out by registered nurses, licensed practical nurses and certified nursing assistants who have experience in geriatric, medical/surgical and rehabilitation nursing.

 


Dining Room Dining room - Patients are encouraged to utilize the dining room for meals and activities on a daily basis. Family is welcome to spend time with their loved ones in the dining room at any time.

 

 

Patient RoomsPatient Rooms - There are only single bedrooms in our facility. This maximizes patient privacy and patient satisfaction.
